Angel:  I am very sorry to hear about your loss.  I want to be respectful of you and your family during this time so am emailing to allow you to respond when you are able.  Many in the community are rallying to create a fund to assist the family with costs for the funeral.  Tremont West has agreed to facilitate this process.  In order to ensure that you and your family have access to these funds in the most efficient way possible, I have spoken with 5th/3rd to establish a memorial fund.  In order to do this, I need some information filled out on a form:

1.       Authorized person to draw on the account

2.       Street address, City and State, zip code, Home Phone

3.       Date of Birth

4.       Mother’s Maiden Name

5.       SS Number

If there is some where we could meet to fill out this information and sign the form, please let me know.  Or we can meet at the bank and have you do it on site.  This account will only be able to be accessed by the signatories of the form.
